本剧情节和17年前Julia Roberts主演的电影《永不妥协》（Erin Brockovich）类似，主人公都是为正义和公理而拼尽全力的年轻女性。Kristin Kreuk在《沉重的真相》中扮演来自大城市的律师Joanna Hanley，她原本是律政界的明日之星，为了承办一起看似简单的案件回到了家乡小镇。她很快发现该案颇具争议且疑点重重，不仅仅是「保护一群患病的女孩」那么简单（有人怀疑一家制药公司的药物导致当地高中女生身体衰弱）。在经历激烈的思想斗争之后，她决定离开大城市的高薪工作，长期留在家乡。与此同时，Joanna少女时代经历的神秘事件开始浮出水面，她终于有机会重新寻找失落的真相……或者说本剧的标题：沉重的真相。该剧并未采用律政剧传统的单元模式，而是标准的连续剧。剧中涉及许多敏感话题，如环境问题、滥用权力问题、家庭暴力问题、社会歧视问题等，还聚焦了女权运动和女性事业成功。
